Heart Failure (HF) is a progressive chronic disease that needs long-term management. It affects 1 – 2 % of the European population and about 10 % of people between 75 and 80 years old. HF incidence is currently increasing, mainly due to innovative therapies and improved survival from myocardial infarction.

In this direction, this consortium presents a proposal for a fully integrated intervention and holistic system, called CareCardia, as an answer to a wide spectrum of parameters like current adoption of the technology, safety, accuracy, effectiveness in terms of patient health, costs and economic aspects, ethical, legal and organisational aspects and, finally, social and cultural aspects.

What is expected by applying CareCardia as a large-scale solution is upgrading the health values system by including a data-value exploitation in its concept through integration and visualisations, is endorsing the empowerment of self-management and the remote health monitoring. Interoperability guarantees an effective and secure connection between the different health systems and the different stakeholders.

CareCardia consortium consists of expertise in the domain of heart failure, mobile and e-health, AI techniques, data integration and digital therapeutic solutions. Through the collaboration and the knowledge exchange of these partners, the design and implementation of a high standards holistic solution is expected.
